Full All-on-4® Rehabilitation: A Stable Bite and a Natural, Lifelike Smile

The Dental Problem

Our 69-year-old patient came to us with several severe and interconnected dental problems. Multiple tooth roots had fractured beyond repair, leaving several teeth non-restorable. In addition, there were extensive gaps, significant teeth grinding (bruxism) and a pronounced deep bite – all of which affected chewing efficiency and long-term bite stability, making everyday eating increasingly difficult. Beyond the functional issues, the condition of the teeth also affected the appearance of the smile – meaning a comprehensive full-arch rehabilitation was needed.

Solution

This two-stage treatment involved restoring both the upper and lower arches using the All-on-4® technique. At the first appointment, we removed the teeth that could not be saved and placed four Nobel Biocare implants in each arch. Temporary All-on-4® bridges were fitted immediately, ensuring the patient could maintain essential function and aesthetics throughout the healing phase.
In the second phase, we designed, fabricated and fitted the final All-on-4® bridges. A key focus during this stage was re-establishing the patient’s bite, as years of deep bite and bruxism had already caused considerable wear and structural damage to the teeth. Ensuring the correct functional bite position was essential – not only to restore comfort, but to prevent overloading and premature wear of the new restorations, supporting long-term stability and durability.

Result

The full treatment journey took approximately five months and resulted in a striking transformation. With the final All-on-4® restorations, the patient now enjoys an aesthetic, well-aligned smile and a stable bite that makes chewing comfortable and effortless once again.
Today, his facial features appear more harmonious and youthful, and the functional compromises that once affected daily life are now firmly in the past.
